Our verdict on AltRx
AltRx is cheap and simple on price, and strict on everything else. Compounded semaglutide is $89 a month and tirzepatide $149, the price includes the clinician, the medication, supplies and shipping, and AltRx says it does not rise with your dose.
The trade-offs are significant. All payments are final, multi-month plans cannot be refunded even in part, FDA sent the company a warning letter about its marketing in June 2026, and Trustpilot has removed its profile. TotalRx costs more month to month, but it refunds you in full if a clinician does not approve you and refunds unshipped months if you stop a longer plan, which is why it ranks above AltRx with us.

Over twelve months on the monthly plan, compounded semaglutide comes to $1,068 and tirzepatide to $1,788. AltRx's brand-name prices are several times the $149 to $449 a month that Novo Nordisk and Lilly charge self-pay customers directly, so brand-name buyers will usually do better elsewhere.
What to know before you choose AltRx
Can I get a refund from AltRx?
Rarely. AltRx's refund policy says all subscription payments are final at the time of purchase. The only exception is if a provider medically disqualifies you, and even then the refund is reduced by the services already provided. Multi-month plans are non-refundable in whole and in part. TotalRx refunds you in full if a clinician does not approve you, and refunds months not yet shipped if you stop a longer plan.
What was the FDA warning letter about?
FDA's letter of June 8, 2026, addressed to Trinity HealthCare Supply, LLC doing business as altRx, said the company made false or misleading claims about its compounded semaglutide and tirzepatide, including product labels suggesting altRx was the maker and claims such as "clinically proven weight loss".
How much notice do I need to cancel?
AltRx's refund policy asks for 72 hours before your billing date; its terms say 48 hours. Allow 72 to be safe, and cancel by email, phone or the patient portal.
What AltRx customers say
With no Trustpilot page to read, we looked at BBB complaints and what customers say elsewhere, summarised in our words.
With no Trustpilot page, customer feedback is harder to find. What remains in search results and BBB complaints is mostly about what was shipped and what was charged.
What people praise
- A low monthly price compared with most programs
- A price that does not rise with the dose
What people complain about
- A different medication or a lower dose shipped than expected
- Learning only after paying that compounded medication is not FDA-approved
- Charges in the second month above what was expected
- Refunds refused under the all-sales-final policy
Treat AltRx's low price as the start of your research, not the end. Its refund policy means a problem after you pay is yours to absorb.
Is AltRx legit?
AltRx is a real telehealth service, but it carries more warning signs than most programs we review. Clinical care is provided by an independent network of medical practices named on its website, and prescriptions are filled by four state-licensed 503A compounding pharmacies it lists in its FAQ. FDA issued a warning letter to Trinity HealthCare Supply, LLC dba altRx on June 8, 2026 over misleading marketing claims; Trustpilot has removed AltRx's profile for breaching its guidelines; and the company's legal name differs between its terms, the FDA letter and its BBB listing. Its compounded medications are not FDA-approved.
Who AltRx suits
- Price is your deciding factor and you want one of the lowest flat rates
- You are comfortable with a strict no-refund policy
- You will stay month to month rather than prepay
