The cheapest semaglutide online, judged by what you pay in a year

Semaglutide adverts compete on the first payment: $49, $79, $99. What decides the cost is every payment after it. We added up a full first year at seven programs, charted where the price jumps, and ranked them on the price you can count on.

Our picks at a glance

Ranked on what a year really costs and how sure you can be of the price, not on the lowest number in an advert. Each line shows the first-year cost on the program's cheapest published plan, and the one thing to check before you pay.

The first month is not the price

Almost every cheap semaglutide offer is built around the first payment. The chart shows what each program charges in month one, and what it charges after that on the same plan. A single dot means the price does not move.

First monthLater months, if different
TotalRx$129 every month
Embody$69 every month
AltRx$89 every month
MEDVi$179 then $299
Eden$138 then $198
WellMedr$49 per 21 days
Hers$188 then up to $448
$0$150$300$450

Lowest published plan for each program. Hers is shown for the Wegovy® pill: the starting dose with membership in month one, the usual dose with membership later. WellMedr's sale price is converted to a calendar month.

The programs with a single dot fall into two groups. Embody and AltRx are cheaper than TotalRx, but treat every payment as final. TotalRx is the lowest price on the chart that is locked for life and comes with a full refund if you are not approved, which is why it ranks first.

Five ways a cheap semaglutide price gets expensive

  1. The introductory month

A low first payment, then a much higher refill. $179 becomes $299 at MEDVi; SkinnyRx shows its $99 first month and the rest at sign-up.

Ask: what will month two cost, in writing?
  2. The membership on top

A separate monthly fee for access to the clinician, charged on top of the medication. At Hers it is $149 a month after the first month, $1,678 in a year.

Ask: is there any fee besides the medication?
  3. Pricing by dose

Semaglutide starts at 0.25 mg and usually steps up every four weeks. If the price follows the dose, your first month is the cheapest you will pay.

Ask: what will I pay at 1.7 mg or 2.4 mg?
  4. The short billing cycle

A plan billed every 21 or 28 days charges you 13 to 18 times a year instead of 12. WellMedr's $49 is per 21-day shipment.

Ask: how many times will I be charged in a year?
  5. The prepaid plan you cannot get back

Longer plans cost less per month, but with an "all payments final" policy, a 12-month plan is lost if you are not approved or need to stop.

Ask: what is refunded if I stop after two months?

Each pick, and the catch

1TotalRx

The lowest price you can lock in

What you pay
$129 a month on the 12-month plan ($1,548), $139 on 6 months, $149 on 3, or $199 month to month. No membership, and the same price at every dose.
Why it is here
Every plan's price is locked for life, so it cannot rise at renewal or as your dose goes up. You get a full refund if a clinician does not approve you, and unshipped months back if you stop a longer plan. Unlimited clinician follow-ups are included.
The catch
It is compounded semaglutide only, as a weekly injection. The lowest rate is on the 12-month plan, paid up front.
2Embody

Lowest 12-month price

What you pay
$79 for one month, $76 a month on 3 months, or $69 a month on a 12-month plan ($828). No membership, and the price does not rise with your dose.
Why it is here
It is the lowest full-year price we found for compounded semaglutide, with flat pricing customers like.
The catch
All subscription payments are final and prepaid plans cannot be refunded. Its terms list different, higher prices for some programs than its store does, and it is only available in some states.
3AltRx

Lowest flat monthly price

What you pay
$89 a month, including the clinician, medication, supplies and shipping. No membership, and AltRx says the price does not rise with your dose.
Why it is here
For people who want to pay month by month without committing, $89 is about as low as an established program goes.
The catch
All payments are final and multi-month plans cannot be refunded. FDA sent the company a warning letter about its marketing in June 2026, and Trustpilot has removed its profile.
4MEDVi

Most support included

What you pay
$179 for the first month of injections, then $299 a month. Tablets start at $249. No membership on compounded plans.
Why it is here
MEDVi bundles dietitian visits, care coaching and unlimited appointments and messaging, and its $299 refill does not rise as your dose does.
The catch
The jump after month one. A year costs more than twice TotalRx's 12-month plan. FDA sent MEDVi a warning letter about its marketing in February 2026.
5Eden

Same price at every dose

What you pay
Compounded semaglutide from $99 a month plus a membership of $39 in the first month and $99 after, as recently listed: about $138, then $198 a month.
Why it is here
Eden is available in all 50 states and DC, and the medication price stays the same at every dose.
The catch
Offers differ depending on where you sign up, so the price you see may not match. FDA sent Eden a warning letter about its marketing in June 2026.
6WellMedr

Lowest sale price

What you pay
From $49 per shipment during its current sale, billed every 21 days. That is up to 18 charges a year, about $74 a calendar month. Regular prices are not published.
Why it is here
While the sale lasts it is the lowest price we found, and it refunds your first payment in full if a clinician does not approve you.
The catch
The 21-day cycle, the time-limited sale, and a money-back promise on its landing page that its refund policy does not back up. The BBB has flagged a pattern of complaints about delivery and billing.
7Hers

Cheapest brand-name semaglutide

What you pay
The Wegovy® pill from $149 a month and the Wegovy® pen from $199, plus a membership of $39 in the first month and $149 after.
Why it is here
If you want FDA-approved semaglutide rather than a compounded version, the Wegovy® pill through Hers is one of the lowest-cost ways to get it with a prescription included.
The catch
Both the membership and the medication go up. At the usual dose of the pill, $299, the total is about $448 a month.

How we ranked them

We scored each program on five things: the cost of a first year at the dose most people reach (35%), whether the price can rise with your dose, after an introductory month or at renewal (20%), what you get back if you are not approved or stop early (15%), whether the full price is shown before you pay and how often you are charged (15%), and the clinical care included in the price (15%). A low price with no refunds and a fluctuating one both score lower than a slightly higher price you can rely on.

Pay monthly or prepay? The numbers

Longer plans are where the real savings are, if the program refunds what you do not use. Here is what a year costs on each plan length at the two programs with the clearest plan pricing:

PlanTotalRxEmbody
Month to month12 monthly payments$199/mo$2,388 a year$79/mo$948 a year
3 months$149/mo$447 per 3 months$76/mo$228 per 3 months
6 months$139/mo$834 per 6 monthsNot listed
12 months$129/mo$1,548 a year$69/mo$828 a year
If you stop early on a prepaid planUnshipped months refundedNot refunded

At TotalRx, the 12-month plan saves $840 a year over paying month to month, and you keep the protection of a refund for months not yet shipped. At Embody, prepaying saves $120, but you cannot get any of it back. If you are not sure you will stay on treatment for a year, that difference matters more than the monthly figure.

Novo Nordisk sells Wegovy® directly to people paying cash through NovoCare Pharmacy, and the same prices appear on TrumpRx.gov 12. Ozempic® is the same medicine approved for type 2 diabetes, not weight management.

The savings card that brings Wegovy® to $25 a month only applies if you have commercial insurance that covers it 1. On Medicare, the GLP-1 Bridge offers Wegovy® for $50 a month to people who qualify 3. Our guide to GLP-1 costs without insurance covers every route.

Is cheap compounded semaglutide safe?

A low price is not a warning sign on its own, since every program here buys from licensed pharmacies. But compounded semaglutide is a different product from Wegovy®, and some checks matter more than the price:

In the STEP 1 trial of brand-name semaglutide 2.4 mg, people lost an average of about 15% of their body weight over 68 weeks 7. There are no trials of that kind for compounded versions. Our compounded vs brand-name guide goes further.

Questions and answers

What is the cheapest semaglutide online?

On a full year, Embody's 12-month plan is the lowest at $828, and AltRx is $89 month to month. Both treat payments as final. TotalRx is $129 a month on its 12-month plan with the price locked for life and a refund if you are not approved, which is why we rank it first.

Is $49 semaglutide real?

WellMedr advertises compounded semaglutide from $49 during a sale, but that is per shipment every 21 days, which can mean 18 charges a year. Its regular prices are not published. Check how often you will be charged and what happens when the sale ends.

Why does my semaglutide price go up after the first month?

Usually because of an introductory first-month price, a membership that rises after month one, or pricing by dose. Programs that charge one flat price, such as TotalRx, Embody and AltRx, do not change it as your dose goes up.

Is compounded semaglutide the same as Wegovy?

No. Wegovy® is made by Novo Nordisk and approved by the FDA. Compounded semaglutide is made by compounding pharmacies, is not FDA-approved, and has not been through the FDA's review of safety, effectiveness or quality.

What is the cheapest way to get Wegovy without insurance?

The Wegovy® pill at $149 a month at the starting dose, or the pen at $199 for the first two fills, self-pay through NovoCare Pharmacy. Both rise at higher doses: $299 for the pill and $349 for the pen at the usual dose. You also need a prescription, which some programs charge a membership for.

Can I switch to a cheaper semaglutide provider partway through?

Yes. A new provider will want your current dose and recent history so you do not restart at the lowest dose. Our guide to switching providers explains how to avoid a gap.

Can I pay for semaglutide with an HSA or FSA?

Usually, when it is prescribed to treat a diagnosed condition such as obesity. Some plans want a letter of medical necessity, and membership fees are less clear-cut.
